| If you’re a Social Worker |

| If you’re not either of these |

• Please fill out the top half of the form with your details • Complete some of the outcome boxes that you feel applies to your situation • Either give the slip to your social worker if you have one, or place it in the repeat prescription box at your GP surgery. You do not need an appointment to discuss this with your GP. • Your GP or social worker will send the completed slip to us, if they do give it back to you, then you can post it or pop it into: Taking Part Workshops Wallsend Town Hall High Street East Wallsend Tyne and Wear NE28 7RR • We will be in contact with you usually within 2 weeks. | What happens next? |

• Taking Part Workshops receives the slip (can take up to 2 weeks) • A member of staff will call the person on the top of the form and go through some questions to help make a good choice about the activity to participate in. • They will answer any questions about the activity or process. • Following that, the member of staff will meet the participant at the chosen activity and complete some paperwork with the participant. • Taking Part Workshops will provide feedback to the referrer about sessions attended, and any experience/ comments the clients consent for us to share.
